Completed enrollment forms w/ parent signature(s)

Immunization Record (Always obtain an updated form before the current form is expired. If the parent cannot provide the form, the child cannot return until the form is updated).

Health Record/Physical (Good for years from the date in the right-hand corner, must be obtained from all children. If the parent cannot provide the form, the child cannot return until the form is updated).
